Please answer the following questions in full. This information serves to ensure transparency and as a basis for possible cooperation.
1. Organisation & Registration
- Official name of the CBO/SHG
- Registration number and issuing authority
- Date of establishment
- Statutes/constitution available? (Yes/No, please attach)
2. Management & Members
- Names and contact details of chairperson, secretary, treasurer
- Number of active members
- Are there regular meetings (e.g. monthly)?
3. Activities & Projects
- Most recent completed activities (brief description, with photos/evidence if possible)
- Planned projects for the coming year
- Are there any voluntary activities that are carried out without external funding?
4. Finances
- Official organisation bank account (name of bank & account name)
- Or: Which financial management system is used (e.g. M-Pesa account in the organisation’s name)?
- Previous sources of income (e.g. membership fees, small fundraising campaigns, partners)
5. Project proposal for NADEUM-WIKO
- Project objective (max. 5 sentences)
- Expected impact on the community
- Rough budget (main items with amounts)
- Timeframe (start – end)
6. Transparency & Evidence
- Willingness to submit simple financial and activity reports on a regular basis (Yes/No)
- Willingness to cooperate with local authorities, schools, churches or NGOs (Yes/No)
